PARENT’S NAME: Mr. Emmanual Yao Similao and Mrs. Victoria Awuawu
REASON/BASIS OF ADMITTING HIM: Critical social investigation about her background revealed that he is needy innocent boy. Both parents are deceased.
DATE OF ADMISSION INTO HOME: He was admitted into New Life Orphanage International November 2005 after strict social investigation about him.
HIS IMMEDIATE NEEDS: His immediate needs are food, clothing, shelter, medical care, education and educational materials.
CONDUCT/BEHAVIOR: He is very quiet, respectful, submissive, compromising but Is a slow learner. When he came into the house he had some behavior problems and would take things that weren’t his. He also would beg for money on the streets.
HEALTH STATUS: When he first came into the home he had rashes all over his body, but now is healthy.
EDUCATIONAL BACKGROUND: Academically, he is very poor .His educational background was very poor but since he came into the home, he is improving gradually. He has repeated a grade. Currently he is attending school.
